We have provided a way for you to complete your new patient forms prior to arriving for your appointment. Simply print the forms, complete them and bring them to your Visalia dental appointment for presentation at our front desk.
Insurance
As a courtesy to our patients, we accept most dental insurance plans and
will gladly file the claim. Co-payments and deductibles (the portion
the insurance does not cover) are due at the time services are rendered.
In order to process claims properly, patients must provide us their
correct and appropriate insurance information and/or inform us of any
changes regarding their dental insurance at the time of service. Please
keep in mind, most dental insurances do not cover cosmetic procedures.
